Endosteal implants to replace permanent teeth for members age 18 or older for up to a $5,000 per member lifetime maximum. These services are covered once for each tooth replacement as long as it is an alternative to a three-unit bridge when the natural teeth and supporting tissue on either side are clinically sound and there are no medical factors or conditions that would affect the success of the implant or that would adversely affect long-term oral health and when a complete lower denture is not clinically appropriate.
