Nicotine gum or nicotine patches (or other smoking cessation aids that require a prescription by law) when prescribed by a Physician. These benefits are limited to $225 for nicotine gum and $200 for nicotine patches and 90-day lifetime supply for other covered smoking cessation aids (or effective June 1, 2002, up to a 90-day supply for each member in each calendar year for other covered smoking cessation aids).
