These benefits are provided only when: the patient has a terminal illness and is expected to live six months or less (as certified by a Physician); the patient and attending Physician have agreed to a plan of care that stresses pain control and symptom relief rather than curative treatment; an adult is the primary care person in the home; and the patient lives in the service area of the hospice provider.
