Routine hearing exams furnished by physician (who is an otologist or otolaryingologist) or by an audiologist under the supervision of a physician for the number of visits or dollar amount described in the benefit limit. This includes: one hearing test to determine if a hearing aid is suitable; and one hearing aid evaluation test.
