Covered Drugs to Treat Asthma, Diabetes, or Coronary Artery Disease (and Depression Associated With Any of These Conditions), or for Smoking Cessation: For these conditions, a lower cost share amount may apply for covered prescription drugs for a member who is age 17 or under and for a member who is age 18 or older who has completed a personal health assessment (PHA). For more information about filling out the personal health assessment or to find out if the completed personal health assessment has been received, the member should contact the plan sponsor. In addition, to be eligible for the full coverage as shown for smoking cessation drugs and covered aids, a member who is age 18 or older must also agree to enroll in an approved smoking cessation program. (All members age 17 or under are automatically eligible for this prescription drug coverage.)
