Hearing aids and related covered services that are furnished by a covered provider (such as an Audiologist or Licensed Hearing Instrument Specialist) for each member up to the amount described in the benefit limit. These covered services include the initial hearing aid evaluation; one hearing aid for each hearing-impaired ear; fitting and adjustments of the hearing aid; and supplies such as ear molds. For members age 22 and older, covered services include batteries.
