Wellness Participation Program - Under a separate wellness participation program, each eligible employee and/or spouse may be reimbursed up to a combined $600 for costs paid for health club or qualified independent fitness studio membership fees, aerobic or fitness classes at a qualified health club or personal training sessions by certified trainers, provided 50 visits are completed by the eligible participant during each six-month period in which the reimbursement is claimed; and up to a separate $150 for each membership each calendar year for costs paid for participating in hospital-based weight loss programs or non-hospital-based weight loss programs designated by Blue Cross Blue Shield. This is the maximum reimbursement for any combination of weight loss program fees incurred by eligible wellness participants enrolled under the same family plan.
