Note: For covered transplants, benefits are also provided for transportation and lodging costs for up to $200 each day, but no more than a $15,000 lifetime benefit maximum for each eligible person. These benefits are provided for the transplant recipient who is a member and one companion (or two companions if the transplant recipient is an enrolled minor dependent child). The companion(s) must travel on the same day as the transplant recipient.
