This health plan covers outpatient treatment of non-biologically based mental conditions, including drug addiction and alcoholism, up to the number of visits described in the benefit limit. This includes services furnished by a Community Health Center (that is a part of a General Hospital), Day Care Center, Detoxification Facility, General Hospital, Clinical Specialist in Psychiatric and Mental Health Nursing, Licensed Independent Clinical Social Worker, Licensed Mental Health Counselor, Mental Health Center, Mental Hospital, Physician or Psychologist.
