This health plan covers outpatient treatment of non-biologically based mental conditions. This includes services furnished by a Community Health Center (that is a part of a General Hospital), Day Care Center, General Hospital, Clinical Specialist in Psychiatric and Mental Health Nursing, Licensed Independent Clinical Social Worker, Licensed Mental Health Counselor, Mental Health Center, Mental Hospital, Physician, Psychologist, Licensed Marriage and Family Therapist or Psychiatric Nurse Practitioner.
