Mental Health Providers Outside of Massachusetts - When covered services are furnished by a mental health provider that does not have a payment agreement with the local Blue Cross and/or Blue Shield plan, each member must pay a separate $240 deductible each calendar year (instead of any overall deductible) and 50% coinsurance.
