Obstructive Sleep Apnea - This health plan covers full in-network and out-of-network inpatient and outpatient services that are furnished by a covered provider for the diagnosis and/or treatment of obstructive sleep apnea and other sleep disorders or sleep-related conditions. These covered services include, but are not limited to: diagnostic tests and services such as in-laboratory sleep studies, non-surgical treatment such as positive airway pressure treatment, the use of continuous positive airway pressure devices such as CPAP, the use of oral appliances and surgical interventions. When covered services are furnished for the diagnosis and/or treatment of obstructive sleep apnea and other sleep disorders or sleep-related conditions, the deductible (unless the member is enrolled in a High Deductible Health Plan with a health savings account), copayment and coinsurance (if there is any) will be waived.
