Methadone maintenance treatment for opioid dependence from an opioid treatment program provider. A referral or prior approval is not required. For these covered services, the member's cost share amount, such as deductible, copayment and/or coinsurance, is waived. There is one exception. If the health plan is a Saver high deductible health plan, the overall deductible will still apply to these services.
