Fitness Program - you and your covered family members can be reimbursed up to $150 per member ($300 per family) for fees paid to qualified fitness programs approved by your employer including gym membership; fitness studio classes; home fitness equipment; or wearable fitness trackers. You can request this reimbursement each calendar year; requests must be submitted by March 31 of the following year. This reimbursement is a Wellness Participation Program separate from your health plan.
