Fitness Program - you and your covered spouse (individually or combined) can be reimbursed a total of up to $600 for membership and class fees paid to a health club with a variety of cardiovascular and strength-training equipment; a fitness studio that provides instructor-led group classes for cardiovascular and strength-training programs; personal training sessions. Reimbursement can be received in each six-month period when 50 health club visits are completed. You cannot be reimbursed for initiation fees, or fees for country clubs, social clubs (like ski, tennis, hiking), sports teams, spas, instructional dance, pool-only facilities, gymnastics, martial arts schools, fitness equipment or clothing, other activities not listed as reimbursable. This reimbursement is a Wellness Participation Program separate from your health plan.
