Weight Loss Program - you and your covered family members can be reimbursed up to $150 (in total) for participation fees paid to hospital-based weight loss programs, and Weight Watchers in-person. You can request this reimbursement each calendar year; requests must be submitted by March 31 of the following year. You cannot be reimbursed for initiation fees; pre-packaged meals, supplements, books, scales, exercise equipment; or individual nutrition counseling sessions, doctor/nurse visits, lab tests and other services that are covered benefits under your medical plan. This reimbursement is a Wellness Participation Program separate from your health plan.
