Under the direction of a physician, a multiple embryo transfer without having previously received a single embryo transfer for members who meet medical necessity criteria (the IVF protocol no longer includes the single embryo transfer requirement during any IVF treatment cycle). A pre-service approval is required from Blue Cross and Blue Shield in order to receive these treatments.
