Additional Wellness Program - you or one of your covered family members may be reimbursed up to $150 year for fees paid to health related activities approved by your employer. Qualifying activities may include fitness programs, smoking cessation, nutritional counseling (not covered by your health plan), weight loss programs, and at home fitness instruction or equipment. You can request this reimbursement each calendar year; requests must be submitted to your employer group by March 31 of the following year. For information about how to become eligible for this reimbursement or for questions about approved health-related activities, you must contact your employer group. This reimbursement is a Wellness Participation Program separate from your health plan.
