Up to an additional lifetime benefit of $3,500 for alcoholism rehabilitation furnished by an Alcoholism Day Treatment Facility. These services include intensive rehabilitative services (individual, group and family therapy) and follow-up services (weekly therapy sessions). Prior approval is required from Blue Cross Blue Shield. (If a copayment, deductible or coinsurance normally applies, these member costs are waived for these covered services.)
