Note: Except for Medicare-approved physical therapy furnished by an independent physical therapist, these Medex benefits are provided only for services that are furnished within 100 days after the member is discharged from an inpatient hospital stay that lasted for at least three days in a row. And, these services must be required to continue active treatment of the condition for which the member was an inpatient.
