This health plan covers outpatient care to treat a medical condition when the services are furnished by the member's PCP or, when arranged by the member's PCP, by another network provider, including a Nurse Practitioner, an Optometrist or a Licensed Dietitian Nutritionist. Or, the member may self-refer for services by a General, Chronic Disease or Rehabilitation Hospital, Community Health Center, Physician, Nurse Practitioner, Nurse Midwife, Optometrist or Licensed Dietitian Nutritionist.
