Outpatient services furnished by a covered Community Health Center (that is part of a General Hospital), General Hospital, Physician (who is a specialist in psychiatry), Psychologist, Licensed Independent Clinical Social Worker, Clinical Specialist in Psychiatric and Mental Health Nursing, Licensed Mental Health Counselor, Licensed Alcohol and Drug Counselor or other mental health provider designated by Blue Cross Blue Shield.
