This health plan covers alcoholism outpatient treatment up to the number of visits described in the benefit limit. (Refer also to Mental Health and Substance Use Treatment - Non-Biologically Based Mental Conditions.) This includes services furnished by a Community Health Center (that is a part of a General Hospital), Day Care Center, Detoxification Facility, General Hospital, Clinical Specialist in Psychiatric and Mental Health Nursing, Licensed Independent Clinical Social Worker, Licensed Mental Health Counselor, Mental Health Center, Mental Hospital, Physician or Psychologist.
