One vision exam following covered eye surgery where the natural lens of the eye is replaced with an intraocular lens and the exam is furnished by a Physician (who is an ophthalmologist) or by an Optometrist and one pair of prescription eyeglasses or contact lenses. For these covered services, the member must pay 20% coinsurance.
