This includes services furnished by a hospital, including a Mental Hospital, Physician, Psychologist or other Medicare covered mental health provider, including a Licensed Independent Clinical Social Worker or by a Clinical Specialist in Psychiatric and Mental Health Nursing or Licensed Mental Health Counselor.
