Nicotine gum or nicotine patches (or other smoking cessation aids that require a prescription by law) when prescribed by a Physician. These benefits are limited to $225 for nicotine gum and $200 for nicotine patches up to a 90-day supply for each member in each calendar year for other covered smoking cessation aids.
