Hearing aids and related covered services that are furnished by a covered provider (such as an Audiologist or Licensed Hearing Instrument Specialist) for members age 21 or under. This coverage for the hearing aid device itself is provided up to the amount described in the benefit limit (the benefit limit does not apply for related covered services). These covered services include: the initial hearing aid evaluation; one hearing aid for each hearing-impaired ear; fitting and adjustments of the hearing aid; and supplies such as ear molds. No benefits are provided for replacement hearing aid batteries.
