Fitness Benefit - This health plan will reimburse costs the member pays for monthly membership fees for three consecutive months of one family or individual health club membership or, as an alternative, for up to 10 fitness classes taken by any combination of subscriber, spouse, and/or dependent children enrolled under the same Blue Cross Blue Shield plan, at a qualified health club during a calendar year. This fitness benefit can be claimed once each calendar year. For more information, refer to policy.
