This Medex plan covers hearing aids and related covered services furnished by a Licensed Hearing Instrument Specialist. For the hearing aid device itself, this coverage is provided up to the amount described in the benefit limit (the benefit limit does not apply for related covered services). These covered services include: the initial hearing aid evaluation; one hearing aid for each hearing-impaired ear; fitting and adjustments of the hearing aid; and supplies such as ear molds. No benefits are provided for replacement hearing aid batteries.
