These home health care benefits may also include: services furnished by a covered Physician; durable medical equipment; portable x-rays and EKGs; and lab tests.

Leg, arm, back and neck braces; trusses and artifical legs, arms and eyes, including adjustments, repairs and replacements required because of breakage, wear, loss or a change in the patient's physical condition.

A routine sigmoidoscopy or barium enema once every three years for a member age 50 or older.

A routine colonoscopy once every ten years for a member age 50 or older.

One visit each group plan year for ages 2 through 5.

Food products modified to be low protein that are medically necessary to treat inherited diseases of amino acids and organic acids for up to $2,500 for each member in each group plan year. The member may buy these food products directly from a distributor.
