This health plan covers medically necessary short-term rehabilitation therapy furnished by a General, Chronic Disease or Rehabilitation Hospital, Community Health Center and Physical Therapist. These covered services include:
This Medex plan covers Medicare-approved ambulance transport to an emergency medical facility for emergency medical care. For example, covered ambulance services include transport from an accident scene or to a hospital due to symptoms of a heart attack. (Refer to the Medicare handbook for more information.)
This Medex plan covers blood storage fees that are eligible for benefits under Medicare. (Refer to the Medicare handbook for more information about Medicare benefits for blood and related fees.)
This Medex plan also covers Medicare-approved observation stays. (Note: These services do not have to be furnished within 100 days of discharge from an inpatient stay to receive these Medex benefits.)
An additional $1,000 in each contract year for inpatient alcohol detoxification services, less any Medicare payment. (Contract year means the 12-month period of time that starts on the group's effective date of coverage and continues for 12 months in a row. A new contract years begins with each 12-month period thereafter.)
Dentists' inpatient services for surgical removal of impacted or unerupted teeth imbedded in bone and removal of seven or more permanent teeth. These benefits also include covered hospital inpatient services when the inpatient stay is not covered by Medicare.
