Periodontics such as:

Periodontal scaling and root planing (once in each quadrant each 24 months).

Periodontal surgery (once in each quadrant each 36 months).

Periodontal maintenance following active periodontal therapy (once each three months).

The member's copayment is waived for surgery or when the visit is only for x-rays.

A sigmoidoscopy or barium enema once every three years for a member age 50 or older.

A colonoscopy once every ten years for a member age 50 or older.

The following age-based schedule is recommended if the member chooses to self-refer for routine care:

One visit every two calendar years for ages 12 through 17.
