This health plan covers cardiac rehabilitation therapy furnished by a covered provider for a member who has had a heart attack (acute myocardial infarction) in the last 12 months or has had coronary bypass surgery (including angioplasty or cardiovascular surgery) or has stable angina pectoris.
This health plan covers Medicare covered chiropractic services furnished by a covered Chiropractor. These benefits are limited to manual manipulation of the spine to correct a subluxation that can be shown by x-ray.
Note: These benefits are also provided when dialysis services are furnished by a non-network provider while the member is temporarily outside the service area (for up to six months in a row).
This health plan provides world wide emergency coverage. This means that this health plan covers emergency medical services whether the member is in the service area or traveling out of the service area. These emergency medical services may include inpatient or outpatient services by providers qualified to furnish emergency medical care and that are needed to evaluate or stabilize the member's emergency medical condition.
These benefits also include full coverage for the first vision exam after eye surgery when the services are furnished by an ophthalmologist or optometrist, including the first pair of contact lenses (or one pair of eyeglasses instead).
