This health plan covers routine mammograms furnished by a General Hospital, Community Health Center, Physician, Nurse Practitioner, Nurse Midwife or Independent Lab.These preventive benefits are limited to one routine mammogram in each calendar year for a member who is age 40 or older.

Medicare-approved hormone replacement therapy for peri- and post-menopausal women; and Medicare-approved monitoring and medical management for members taking psychiatric drugs and neuropsychological assessment. (Note: These services do not have to be furnished within 100 days of discharge from an inpatient stay to receive these Medex benefits.)
