Routine mammograms (limited to: one baseline mammogram during the five-year period a member is age 35 through 39; and one every year for members age 40 and older).
Immunizations, including: pneumococcal pneumonia vaccine and its administration; hepatitis B vaccine and its administration for members at risk; other vaccines for those at risk (such as anti-rabies vaccine for those possibly exposed to rabies); influenza vaccine and its administration (flu shots); and travel immunizations.
Colorectal cancer screening. This includes: a fecal-occult blood test every 12 months for members age 50 and older; a flexible sigmoidoscopy every 48 months for members age 50 and older; a colonoscopy every 24 months for members who are at high risk for developing colorectal cancer; and other colorectal cancer screening tests and procedures according to the limits and/or requirements set by Medicare.
Prostate cancer screening. This includes one digital rectal exam and one prostate specific antigen (PSA) blood test each year for members age 50 and older.
Fitness Benefit. Under this fitness benefit, up to a total of $150 each calendar year will be reimbursed for fees that the member paid for a health club membership or for fitness classes at a health club. (Refer to policy for more information about expenses that qualify for this benefit.)
This health plan covers one routine GYN exam, including a routine Pap smear test, once each calendar year when the services are furnished by a covered provider, including a Nurse Midwife or Nurse Practitioner.
