One routine PSA (prostate-specific antigen) blood test each group plan year for a member age 40 or older.

A sigmoidoscopy or barium enema once every three years for a member age 50 or older.

A colonoscopy once every ten years for a member age 50 or older.

One exam every five years from age 19 through age 29.

One exam every three years from age 30 through age 39.

One exam every two years from age 40 through age 54.

One exam each group plan year for a member age 55 or older.

Food products modified to be low protein that are medically necessary to treat inherited diseases of amino acids and organic acids for up to $2,500 for each member in each group plan year. The member may buy these food products directly from a distributor.

Nicotine gum or nicotine patches (or other smoking cessation aids that require a prescription by law) when prescribed by a Physician. These benefits are limited to a 90-day supply for each member in each group plan year.
