This Medex plan covers Medicare-approved outpatient diagnostic tests furnished by a Medicare covered provider, including a Nurse Practitioner. These benefits include:

Diagnostic lab tests.

Diagnostic x-rays and other imaging tests.

Other diagnostic tests.

Medex pays the Medicare Part B deductible and Part B coinsurance.

This Medex plan covers medical care services to diagnose and/or treat an illness or injury when furnished by a Medicare covered provider, including a Nurse Practitioner. These benefits include:

Clinic, home and office visits. (Refer to Prescription Drugs for more information about chemotherapy and other drugs that are administered by a Medicare covered provider.)

Follow-up medical care related to an accidental injury or medical emergency.

Medical exams to fit prosthetic lenses that are covered by Medicare.

Non-dental services furnished by a Dentist only if the services would normally be covered when furnished by a Physician.
