This health plan also covers one hearing aid (or one set of binaural hearing aids) for up to the benefit limit for each calendar year for a member who is age 19 or under. This includes dispensing fees; acquisition costs; batteries; replacement parts; charges to replace lost or broken hearing aids; and repair of hearing aids. (These benefits do not count toward any benefit limit that may apply to durable medical equipment.)

Two visits for age 2.

One visit each calendar year for ages 3 through 18.

The member's copayment is waived when there are no in-network free-standing radiology facilities within a 30-mile radius of where the member lives; or when the services are furnished during an inpatient stay or in conjunction with a surgical procedure on the same day; or when the services are furnished during an emergency room visit at the same facility; or when the services are furnished during observation at the same facility.

Weight Loss Program Benefit - All members may take part of the weight loss program benefit. For each membership, up to a total of $150 each calendar year is reimbursed for fees paid for hospital-based weight loss programs or weight loss programs designated by Blue Cross Blue Shield which includes Sensible Nutrition Connection. The member can claim this maximum benefit for any combination of fees incurred by the subscriber and/or other members enrolled under the same family membership.

Nicotine gum or nicotine patches (or other smoking cessation aids that require a prescription by law) when prescribed by a Physician.

Generic vitamins that require a prescription.

Routine sigmoidoscopies, barium enemas, colonoscopies and IUDs.

Surgical services for obesity include skin reduction surgery (panniculectomy) when the surgery is authorized in advance by Blue Cross Blue Shield.

Note: The member's deductible is waived for the following prenatal tests: bacteriuria; RH incompatibility; and other prenatal care as may be recommended by the Health Resources and Services Administration.
