Special Note: Under this program, if the member is admitted directly from the emergency room into a hospital that is not a Blue Distinction Center, the member will pay the same cost share amount that he or she would pay at a Blue Distinction Center.

$15,000 lifetime maximum per eligible member per condition.

$5,000 lifetime maximum per eligible member per condition.

Bariatric surgery.

Cardiac care.

$10,000 lifetime maximum per eligible member per condition.

Complex and rare cancers.

Knee and hip replacement.

Spine surgery.
