This health plan covers medically necessary outpatient cardiac rehabilitation furnished by a General or Chronic Disease Hospital or Cardiac Rehabilitation Provider up to the amount described in the benefit limit.
Pediatric care furnished by a Physician (who is a pediatrician) or Nurse Practitioner for a well newborn for up to seven visits. (These seven visits include any subsequent visits for outpatient routine pediatric care received during the first year of life. Refer to Routine Pediatric Care.)
For drugs that are not covered by Medicare, this Medex plan covers drug and supplies that are prescribed by a physician for outpatient use and purchased from a Medex covered pharmacy up to the dollar amount described in the benefit limit. These covered drugs and supplies include:
